Insurance Information

We participate with all the major insurance carriers and workman's compensation groups.  If your insurance carrier allows; you may contact us directly for an appointment and we will do the paperwork.  If you belong to an HMO;  we will need a referral from your primary care physician (gatekeeper) in order to comply with your contract.  For uninsured or individuals with high deductibles; we will discuss options, costs, and payment schedules.
